Purpose of Veteran Consultations

Torii Health provides consultations for Veterans who would like an independent medical perspective on their current medical conditions and how those conditions are documented in the medical record.

Many Veterans experience difficulty navigating the disability evaluation process, particularly when living outside the United States where access to VA facilities and documentation systems may be limited.

Our consultations focus on:

  • Reviewing available medical records

  • Clarifying diagnoses and functional limitations

  • Identifying gaps or inconsistencies in the medical record

  • Discussing what types of documentation may be helpful when pursuing a claim or reevaluation

The goal of these consultations is to provide clinical clarity and practical guidance, not to produce predetermined documentation.

Scope of Services

Veteran consultations may include:

  • Review of medical history and symptoms

  • Review of available imaging, laboratory results, and medical notes

  • Discussion of service-related injuries or exposures

  • Evaluation of functional limitations related to diagnosed conditions

  • Guidance on documentation and care options

Consultations may be conducted in person or via secure telemedicine, depending on clinical appropriateness.

Important Limitations

Torii Health does not provide pre-arranged nexus letters or independent DBQs.

Medical opinions or supporting documentation are only provided when:

  1. The medical record supports the conclusion, and

  2. The opinion is consistent with professional clinical judgment.

Many services advertise guaranteed nexus letters or pre-written reports. Torii Health does not operate in this manner. All medical documentation must be medically justified and supported by the available evidence.

Consultation Process

Before scheduling a consultation, patients are asked to provide basic information about their case, including:

  • The condition being evaluated

  • Relevant service history

  • Current diagnoses

  • Available medical records or imaging

  • Any prior VA rating decisions, if applicable

This information helps determine whether a consultation is appropriate and ensures that time is used effectively.

Cases may be declined if sufficient documentation is not available or if the clinical question cannot reasonably be addressed.

Fee Structure

Veteran consultation services are provided on a self-pay basis.

Typical starting fees are as follows:

Initial Veteran Consultation

$200–$250

30–45 minute consultation including preliminary review of available records.

This visit focuses on understanding the medical history, reviewing documentation, and discussing potential next steps.

Medical Record Review or Written Medical Opinion (when appropriate)

$700–$1,200 (depending on the complexity)

This service is only offered when the medical record supports a clinically defensible opinion and when sufficient documentation is available for review.

Written opinions are not guaranteed and are provided only at the physician’s discretion.

Primary Care Evaluation

In some situations, the most appropriate approach may be ongoing medical care to evaluate and treat a condition over time. Documentation that arises through longitudinal care may sometimes provide the most accurate clinical record.

Primary care services are billed according to the clinic’s standard professional fee schedule.

Insurance Participation

Torii Health does not accept Japanese National Health Insurance (Kokumin Hoken).

Veteran consultation services are typically self-pay.

In certain situations, if care is provided as part of routine primary care, U.S. insurance plans may be billed when applicable, subject to the policies of the patient’s insurance carrier.

Records & Documentation

Patients should provide any relevant records prior to consultation when possible.

Helpful documentation may include:

  • Orthopedic or specialist notes

  • Imaging reports (X-ray, MRI, CT)

  • Operative reports

  • VA rating decision letters

  • Prior VA examinations

  • Laboratory results

Providing records in advance allows the consultation to focus on meaningful clinical review.
