• JMA Hospital Card: This card has "JMA" written on it. It is used at both Zama General Hospital and Ebina General Hospital.

• Appointment Form: Provided by our clinic.

• Referral Letter: Provided by our clinic.

• Downloaded Questionnaire: Can be found below. Please print and fill this out before you arrive.

• Registration Form: Only if you do not already have a JMA card.

Step-by-Step Instructions

1. Arrival & Entrance
   Your entrance depends on whether you have visited a JMA hospital before:
   - If you HAVE a JMA Hospital Card: Enter through the West Entrance.
   - If this is your FIRST time (No Card): Enter through the Main Entrance and go to the Main Reception / New Patient Counter to register. Please hand in your filled in registration form. Once you receive your card, follow the signs to the West Building.

2. Check-in (Counter 16)

   At Counter 16, present your JMA card, appointment form, and referral letter to the staff sitting behind the computers.

   Once they have checked your appointment and given you back your card, they will give you a bookelet to take a look at while you wait in the waiting space. Please use the "Hospital Information Booklet" download above as your English reference for this booklet..

3. The Exam (Room 33)

   A female technician will come to the waiting area and call your name. Follow her to Room 33(Radiography/Mammography).

   Once inside, you will take a seat in a chair to answer some questions. She will first ask you to state your full name and date of birth.

   She will ask a few standard questions. Use your completed Questionnaire to help answer these.

4. The Examination

   The technician will ask you to remove all upper clothing. She will position your breasts on the machine. There will be 4 images total (2 views of each breast). During the scan, please follow the instructions of the technician.

5. Completion & Parking

   You are finished! You do not need to pay at the hospital. Results and billing are sent directly to Torii Health.

   If you drove to the hospital, please stop by Counter 6 (Check-out) before leaving to get a discount ticket for parking. To get to Counter 6, exit the radiography room, turn left, walk to the main hall, then turn left again. You can find it on the left side. Show the phrase below to get parking tickets from the people at the desk.

Show these to staff if you need assistance:

• At Check in: I am here for a mammogram and was referred by Iris Dental Medical.
  アイリスデンタルメディカルからマンモグラフィーの検査共同利用のために来ています。

• Parking: I had a mammogram via referral. I was told to come to Counter 6 for my parking ticket.

  本日、検査共同利用を通じてマンモグラフィーを受けました。車を運転してきたため、駐車券をもらうために6番窓口へ寄るよう言われました。
