Mammogram Patient Guide

(Ebina General Hospital)

This guide is for patients referred by Iris Dental Medical/Torii Health for a mammogram at Ebina General Hospital. Please follow the steps below for a smooth experience.

 
 

What to bring with you

  • JMA Hospital Card: This card has "JMA" written on it. It is used at both Zama General Hospital and Ebina General Hospital.

    • If you do NOT have a JMA card: Please bring the Registration Form provided by our clinic.

  • Appointment Form: Provided by our clinic.

  • Referral Letter: Provided by our clinic.

  • Downloaded Questionnaire: Please print and fill this out before your mammogram.

 

Location & Transport

Ebina General Hospital is located approximately 12 minutes on foot from Ebina Station.

  • By Car: On-site parking is available. Please see the Departure & Parking section below for instructions on how to receive a parking discount.

  • Free Shuttle Bus: A free shuttle runs from the Ebina Station East Exit (Sub-Rotary). The ride takes approximately 5 minutes.

Step-by-Step Instructions

  1. Arrival & Entrance
    Your entrance depends on whether you have visited a JMA hospital before:
    - If you HAVE a JMA Hospital Card: Enter through the West Entrance.
    - If this is your FIRST time (No Card): Enter through the Main Entrance and go to the Main Reception / New Patient Counter to register. Please hand in your filled in registration form. Once you receive your card, follow the signs to the West Building.
